Membership Application Information
You must meet the following requirements and be able to show proof of any and/or all information provided.
You must be a US Citizen
You must be at least 20 years of age
You must live with in 30 miles of Omaha or Fremont, Nebraska
You must provide medical information
You must provide a valid drivers license
You must be willing to undergo a drug screening test if requested
To join our organization we have three forms you will need to fill out and e-mail or ground mail back to our organization. We do not allow applications on line due to the risk of identity theft. All forms must be completed in their entirety.
